When someone says spacers are perfectly safe, they mean quality hub-centric spacers with correct hardware, measured thread engagement and proper torque, which is a true statement, and factory engineers agree, since several manufacturers ship cars with spacers from the plant.
When someone says spacers killed a wheel, they mean a soft no-name ring that never centred on the hub, bolts three threads deep, installed with an impact gun in a parking lot, which is also a true story. The argument never resolves because the two sides are not describing the same object. Safety here is not a property of the word spacer, it is a property of the specific part and the specific install.
Hub-centric or nothing. The car's weight should ride on the hub bore, not hang on the wheel studs. A hub-centric spacer picks up the hub lip on one side and reproduces it for the wheel on the other, so everything stays centred and loaded the way the engineers intended. Lug-centric rings that rely on bolt tension alone are the product behind the horror stories.
Thread engagement is math, not vibes. A slip-on spacer eats stud length, and your lug nuts need a minimum number of turns of engagement to be trusted. I count it during the install, against the rule of thumb for your stud size. If the answer is not enough, the fix is extended studs or bolt-on spacers with their own studs, and until then the spacers stay in the box, whatever they cost.
Torque, in stages, then re-checked. Spacer hardware and lug nuts get the torque wrench in stages to spec, you get the values on a card, and everything gets re-checked after the first drive settles it in. This is also the step the parking-lot install always skips, which is why my spacers stay boring and attached.
Honesty corner: sometimes the look you want is not a spacer problem. If you are pushing serious width to fix fitment on wheels whose offset was wrong for the car, the better answer is the right offset on the next set of wheels, and I will say so even though it costs me an install. Spacers shine for fine-tuning: clearing a big brake kit, evening up staggered fitments, or bringing a slightly tucked wheel out to sit right. Ten to twenty millimetres of thoughtful is the sweet spot; forty millimetres of compensation is a flag.

Correctly sized hub-centric spacers, torqued properly, on a balanced setup: no. Vibration after spacers means something in the stack is not centred (a lug-centric ring, a hub face that was never cleaned, a lip not seated) or hardware settled because it was never re-torqued. All of which are install problems, all of which are the exact steps my install refuses to skip.
A sensible spacer changes the wheel's scrub and offset slightly, the same as buying wheels with a different offset, and bearings live with that fine. What genuinely eats bearings is big width pushed way outboard on a heavy vehicle, which is the forty-millimetre-compensation territory I already talk people out of. Modest, measured, torqued: your bearings will not notice.
Yes, no law prohibits them. The legal edge is downstream: the tire still has to stay inside the bodywork or flares, because tires past the fender line fail inspection and the wheel-spray rule. Slip-on with adequate remaining thread is simple and fine, and the measurement decides it. Once a slip-on gets thick enough to starve the threads, bolt-on spacers (which carry their own studs) or extended studs become the correct engineering.
